Liver fat in people with HIV and fatty liver disease

Study question & design
A multicenter, double-blind study compared tesamorelin with placebo over 12 months in people with HIV and non-alcoholic fatty liver disease.
Reported findings
Liver fat decreased more with tesamorelin. The study reported more localized injection-site complaints, while changes in fasting glucose and glycated hemoglobin did not differ between groups.
Limits of interpretation
The trial was small and population-specific. Reduced liver fat does not establish long-term histological benefit or efficacy in every form of fatty liver disease.
Researchers analyzed paired liver biopsies from the earlier randomized trial to investigate gene pathways affected by tesamorelin.
Reported findings
Expression of oxidative-phosphorylation gene sets increased, while several inflammation and cell-division gene sets decreased. Some changes correlated with a fibrosis-related gene score.
Limits of interpretation
These are molecular associations from a trial subset. They do not demonstrate prevention of liver cancer or independently confirm a clinical outcome.
